Our interdisciplinary care includes nursing, physician oversight, social work, spiritual support, hospice aide services, and bereavement support.
Our Hospice Care Team
Our interdisciplinary hospice team focuses on comfort, symptom management, dignity, and quality of life. Care may be provided in the patient’s home, a nursing home, or hospital setting, depending on the patient’s needs and plan of care.
Skilled Nursing Care
Skilled nurses are trained to assist with pain and symptom management and support patients and families throughout hospice care.
Physician Services
The patient’s attending physician participates as part of the hospice team. The hospice Medical Director works with the attending physician, helps certify the prognosis, leads development of the plan of care, and provides physician consultation.
Social Work
Social workers provide counseling and help patients and families identify and access appropriate community resources and support services.
Spiritual Care
Chaplains and spiritual counselors are available to provide spiritual support when desired by the patient and family.
Hospice Aides & Homemakers
Hospice aides and homemakers are part of the care team and provide supportive services consistent with the patient’s individualized plan of care.
Bereavement Support
Bereavement support is available as part of the hospice program to support families and loved ones through grief and loss.
Volunteers
Hospice volunteers may provide companionship and practical assistance through visits, reading, letter writing, shopping, and other supportive activities.
